Mission Statement
Upper Valley Community Health Services, a Community Health Center, makes high quality, culturally sensitive, primary medical, dental and behavioral health services affordable and accessible to the people of Idaho’s Upper Valley region.
UVCHS will provide a physician based, freestanding, 5 day per week, full service clinic in St. Anthony, ID (Fremont County). UVCHS will provide comprehensive primary and preventative medical, dental and mental health services in a culturally appropriate and sensitive manner with a family oriented, life cycle focus to serve the medically unserved and underserved population in the Fremont and Madison County target areas. This includes translation and/or Spanish/English bilingual access for all patients receiving any and all services. We will provide these services on a sliding fee scale. This is determined by family size and income level.
The following services will be offered on-site during office hours and will be provided directly by UVCHS staff: preventative and primary health services, acute and chronic care services, pre-natal and ob/gyn care, referrals for hospitalization and specialty care, preventative dental services as well as dental treatment modalities, behavioral health services provided by a LCSW health education, health promotion and disease prevention (HPDP) medication access, outreach, case management, and referrals to other medical and social service agencies
